You can now upload your COVID-19 booster vaccination information to your Health Portal. While the university has not mandated the COVID-19 booster vaccination, it does strongly encourage students, faculty, and staff to receive it if they are eligible. If you have received the COVID-19 booster vaccination, we would ask that you add this information to your Health Portal. This data will help us as we continue to assess/revise COVID-19 protocols.
To submit proof of your COVID-19 booster vaccination:
- Select “My Forms” and then click “COVID-19 Booster History.”
- Select the type of vaccination and date received.
- Upload an image of your booster vaccination card (i.e., a picture from your phone).
- Click “Submit.”
This portal is FERPA and HIPAA compliant. Faculty, staff, and students can use their Saint Mary’s username and password to log in. If you have difficulty accessing the portal or uploading a document, contact the HelpDesk at email@example.com. Other questions can be directed to firstname.lastname@example.org.
