Update your COVID-19 vaccination information

Did you previously submit an exemption form for the COVID-19 vaccination, but have since been vaccinated? If so, we would ask that you update your information in the Health Portal.  

Please email covid@smumn.edu indicating you need to update your COVID-19 vaccination history. Your submitted exemption form will be deleted and a new COVID-19 vaccination form will be assigned to you. You can then log into the Health Portal, complete that form, and upload proof of your COVID-19 vaccination.

Have you received your COVID-19 vaccination booster? You can log into the Health Portal and report your COVID-19 vaccination booster using the COVID-19 Booster History form. 

Your updated COVID-19 vaccination information and your COVID-19 vaccination booster information are confidential, but this information helps us as we continue to assess/revise COVID-19 protocols.
