The 2021 W-2 forms are available online. You can view your W-2 through your payroll portal. Former employees may access them at http://smumn.greenemployee.com.
The 1095-C form for employees who had or were eligible for health insurance through Saint Mary’s in 2021 will be mailed before January 31.
