The Minnesota Paid Family and Medical Leave (PFML) Acknowledgement Form was posted on Jenzabar on 11/24/2025. This form is required for all employees and completing it helps us make sure that everyone is informed about PFML benefits, rights, and responsibilities.

What You Need to Do:
1. Go to Jenzabar and Login using your SMUMN credentials: https://
2. Select My Payroll on the left Employee dropdown
3. Locate the HR Forms and click on the Minnesota Paid Leave Form
4. Read through the form and select the Electronic Signature Box at the bottom and Submit.
5. After you Submit the form, you should receive a copy of the completed form within 24 hours.
