In the alternative, if you are looking to confirm that your vaccination information/exemption has been received:
- Log in to your health portal and select “My Profile.”
- Select “Immunization History.”
- Download your immunization record.
- If your COVID-19 immunization record/exemption is not immediately visible, please allow five business days and then contact us if it is still not reflected in your immunization history.
