Access Services: Updated Faculty Testing Form

Hi Faculty and Staff,

Updates have been made to the Faculty Testing Form that Access Services uses to gather information from faculty and staff about quizzes and exams that students will take in the Access Services Testing Center. We tried to streamline the form so you wouldn’t be prompted to answer so many questions if they were not necessary for your exam. Please be aware that the form looks different when filling it out this semester. Also, please let Access Services know if you run into any issues when completing the Faculty Testing Form.
