Access Services: Student Testing Form Update

Hi Students,

If you are registered with the office of Access Services and you use the Access Services Testing Center, updates have been made to the Student Testing Form that Access Services uses to gather information from students about exams that you are planning to take in the Access Services Testing Center. We tried to streamline the form so you wouldn’t be prompted to answer so many questions if they were not necessary for your exam request. Please be aware that the form looks different when filling it out this semester. Also, please let Access Services know if you run into any issues when completing the Student Testing Form.
